Man, Woman, Organization & Chamber Member of the Year
Nomination Form
Each year the Parke County Chamber of Commerce recognizes a man, woman,
organization and Chamber Member for their contributions, above and beyond the normal, to the
county. Anyone is eligible to nominate any Parke County resident or
organization in the appropriate category. This can be done by mailing or
faxing in a letter of recommendation or nomination form as published in the
Parke County Sentinel or completing the on-line nomination form below.
Ensure you provide justification for the award as the award recipients are
selected by a panel of past award winners and not by the number of nominations
they receive.
When considering a nomination, please consider the following
criteria:
 | Nominees must be Parke county residents |
| Nominees must have given extended service to the community and/or county
in more than one area |
| Evidence of service to the community on a volunteer basis is considered
important |
| Nominees need not be members of the Parke County Chamber of Commerce |
| Only one name may be submitted for each category by a nominator |
